Application Policy/Procedure
For the 2008-2009 School Year
Grades 1-7 Applications Accepted After February 1st
Deadline for Application: June 1st
Kindergarten Applications Submitted at Registration on March 6, 2008
1. The following must be sent to St. Catherine of Siena School:
Completed application form
Signed release of information for child's school
Signed release of information for child's teacher
Health Form: Proof of Immunization
Birth Certificate (copy)
Baptismal Certificate (copy)
$50.00 Nonrefundable application fee
2. Requests for records will be sent to schools/teachers with return requested as soon as possible.
3. Records will be reviewed as soon as they are received, and a personal interview will be scheduled. Class size, individual academic program and achievement, as well as behavior will be considered in determining acceptance. Members of SCS Parish and Albany Diocesan Catholic parishes are given priority when space is limited.
4. At the completion of the above, parents will be notified of acceptance or non-acceptance into our program, with acceptance finalized with the receipt of $150.00 Registration Fee and the signed Tuition Agreement.
5. Anyone who wishes to go on a waiting list, must submit all of the items listed in step number one (fee will be returned by August 31st, if there is not space).
Additional Procedure for Kindergarten Application
1. We will arrange an appointment in June for your child to be
screened by our Kindergarten teacher. This affords an opportunity for the child and
teacher to get to know each other and an evaluation made of the child's readiness for
Kindergarten.
A final acceptance letter will be mailed after screening.
2. A physical form completed by your child's doctor should be returned by the first day of school, if possible. Physicals must be dated after September 1, 2008.
Registration Information for Families New to SCS Pre-School for 2008-2009
Entering Pre-School 3 or 4 Year Old Program
Parents of 3 & 4 year olds who are new to SCS in September 2008 must attend a short presentation on program, tuition and routines on February 12, 2008 at 7:00 p.m. in the Parish Center.
You should call the school office by February 8, 2008 to request an application, which should be completed and brought to registration. A $100.00 non-refundable Application/Registration Fee is due with the application for all new students entering the 3 and 4 year old classes.
